The Requirements
To meet the basic qualifications for this role you will have legal authorization to work permanently in the United States for any employer without requiring a visa transfer or visa sponsorship. To be a good fit for the Physical Therapist opportunity, you also should have:- A bachelor's degree or higher in Physical Therapy from an accredited program.
- Current licensure as a Physical Therapist by the California Board of Physical Therapy.

- A year of experience as a Physical Therapist is preferred, but we also will consider qualified recent graduates.
- Experience with home health care is preferred, but at a minimum, you should be able to work, visit patients and follow CVHCare procedures with minimal supervision.
- Sufficient computer skills to operate and complete documentation in a timely manner in an electronic medical record (EMR) application.
- The ability to learn Medicare regulations, including how to perform an Outcome and Assessment Information Set (OASIS), within three months.
- Bilingual skills are a plus, but we offer translation services in more than 150 languages.

The Role
Reporting to the Director of Rehabilitation or Rehabilitation Manager, you will join a growing team of Physical Therapists. You also will interact regularly with other members of our multidisciplinary care team, and you may work with PT Assistant on some cases.You'll provide a variety of therapy services to help patients who have been injured or physically affected by illness, to recover and/or improve function. This will include evaluating a patient's condition and developing a customized physical rehabilitation and treatment plan to enhance strength, flexibility, range of motion and motor control, as well as to reduce any pain, discomfort and swelling the patient is experiencing. More specifically, you will:
- Provide physical therapy services to patients according to a written physician's plan of care. This may include, but will not be limited to, assessing and evaluating therapeutic/rehabilitative/functional level of function, and participating in the development of the total plan of care.
- Evaluate home environment.
- Assess for muscle strength, mobility, gait, ROM -- potential for rehab.
- Direct physical therapy treatment.
- Instruct patients and families/caregivers in the use and care of therapeutic appliances.
- Determine priority needs for physical therapy
- Notify physician of patient plan of care (POC) and update on any changes in condition throughout care.
- Initiate physical therapy program and instruct other personnel and/or family/caregivers in certain phases of physical therapy.
- Involve family/caregivers in the development of patient POC.
- Train patient in the use of prosthetic devices as appropriate.
- Identify patient and family/caregiver needs for other home health services and refer as necessary. This will include advising and consulting with the family and other agency personnel.
- Prepare and submit clinical and progress notes and summaries based on the attainment of goals, and revise plan of care as necessary.
- Participate in discharge planning for patient.
- Prepare and submit a clinical progress summary based on the attainment of goals as directed by Organization policy.
- Provide physical therapy consultation to home families/caregivers when indicated.
- Provide in-service education programs for nursing organization personnel as needed, and participate in in-service programs.
- Participate in peer consultation process.
- Supervise Physical Therapy Assistants according to organization policy and state regulations.
- Instruct, supervise and evaluate home health aide care every two (2) weeks in cases where therapy is the only skilled service.

You'll have a good deal of leeway in scheduling your hours and even days, as long as you accommodate patient needs. You can choose to work Monday through Friday, or work one weekend day and have a weekday free. In general you'll work standard hours, but you also could take a two-hour break between morning and afternoon patients, making up those two hours in the early evening.

Our executives take turns in a 24-hour on-call rotation to make themselves available to you at any time. You'll enjoy paid orientation, which includes extensive classroom and hands-on training, and incorporates a simulation skills lab and 1:1 field training with seasoned peers and clinical management. Our dedicated Clinical Training Department offers ongoing learning, and we pay for continuing education and training. If you are new to home health care, we think you will find it very rewarding. At the same time, it is different from working in a clinical setting. Working in the field means you'll need to be resourceful and self-sufficient in solving problems and achieving goals. Flexibility and adaptability are also essential; for example, the schedule you start with in the morning may change during the course of the day. In addition to focusing on the orthopedic aspects of a patient's case, you will have case management responsibilities. This will involve performing a full system review, including taking vitals, assessing safety issues, accounting for conditions like diabetes, and more.We have created a strong support infrastructure, so you won't feel like you are on an island. Just the same, you should be proactive about communicating with patients, families, physicians, other clinicians, your supervisor, your scheduler and other stakeholders. We have tools to help you do so, but it will be up to you to put them to good use. You should be comfortable incorporating technology into your work routine.
