The Requirements

To meet the basic qualifications for this role you will have legal authorization to work permanently in the United States for any employer without requiring a visa transfer or visa sponsorship. To be a good fit for the Physical Therapist opportunity, you also should have:
  • A bachelor's degree or higher in Physical Therapy from an accredited program.
  • Current licensure as a Physical Therapist by the California Board of Physical Therapy.
  • Current Basic Life Support for Healthcare Providers (BLS) or Cardio-Pulmonary Resuscitation/Automated External Defibrillator (CPR/AED) certification. Quick question for you - click here
  • A year of experience as a Physical Therapist is preferred, but we also will consider qualified recent graduates.
  • Experience with home health care is preferred, but at a minimum, you should be able to work, visit patients and follow CVHCare procedures with minimal supervision.
  • Sufficient computer skills to operate and complete documentation in a timely manner in an electronic medical record (EMR) application.
  • The ability to learn Medicare regulations, including how to perform an Outcome and Assessment Information Set (OASIS), within three months.
  • Bilingual skills are a plus, but we offer translation services in more than 150 languages.

We'll expect you to come into the San Ramon or Burlingame office for a monthly meeting (in some situations you can join the meetings virtually), but otherwise you will spend your time in the field at patient homes. Our goal is to minimize your drive time, but you should be prepared to visit patients within 15 to 20 miles of your home. If you do have to travel far, we'll try to assign you to multiple patients in the same area. The Role

Reporting to the Director of Rehabilitation or Rehabilitation Manager, you will join a growing team of Physical Therapists. You also will interact regularly with other members of our multidisciplinary care team, and you may work with PT Assistant on some cases.

You'll provide a variety of therapy services to help patients who have been injured or physically affected by illness, to recover and/or improve function. This will include evaluating a patient's condition and developing a customized physical rehabilitation and treatment plan to enhance strength, flexibility, range of motion and motor control, as well as to reduce any pain, discomfort and swelling the patient is experiencing. More specifically, you will:
  • Provide physical therapy services to patients according to a written physician's plan of care. This may include, but will not be limited to, assessing and evaluating therapeutic/rehabilitative/functional level of function, and participating in the development of the total plan of care.
  • Evaluate home environment.
  • Assess for muscle strength, mobility, gait, ROM -- potential for rehab.
  • Direct physical therapy treatment.
  • Instruct patients and families/caregivers in the use and care of therapeutic appliances.
  • Determine priority needs for physical therapy
  • Notify physician of patient plan of care (POC) and update on any changes in condition throughout care.
  • Initiate physical therapy program and instruct other personnel and/or family/caregivers in certain phases of physical therapy.
  • Involve family/caregivers in the development of patient POC.
  • Train patient in the use of prosthetic devices as appropriate.
  • Identify patient and family/caregiver needs for other home health services and refer as necessary. This will include advising and consulting with the family and other agency personnel.
  • Prepare and submit clinical and progress notes and summaries based on the attainment of goals, and revise plan of care as necessary.
  • Participate in discharge planning for patient.
  • Prepare and submit a clinical progress summary based on the attainment of goals as directed by Organization policy.
  • Provide physical therapy consultation to home families/caregivers when indicated.
  • Provide in-service education programs for nursing organization personnel as needed, and participate in in-service programs.
  • Participate in peer consultation process.
  • Supervise Physical Therapy Assistants according to organization policy and state regulations.
  • Instruct, supervise and evaluate home health aide care every two (2) weeks in cases where therapy is the only skilled service.

In general you'll work standard hours, but you also could take a two-hour break between morning and afternoon patients, making up those two hours in the early evening.

Your supervisors are happy to take calls or meet with you to answer care questions, brainstorm ideas or help clear up confusion. Our executives take turns in a 24-hour on-call rotation to make themselves available to you at any time. You'll enjoy paid orientation, which includes extensive classroom and hands-on training, and incorporates a simulation skills lab and 1:1 field training with seasoned peers and clinical management.
