The Requirements

To meet the basic qualification for this role you will have legal authorization to work permanently in the United States for any employer without requiring a visa transfer or visa sponsorship. To be a good fit for the Certified Home Health Aide opportunity, you also should have:
  • A High School Diploma or equivalent.

  • At least one year of experience working as a Home Health Aide is preferred, but we also will consider recently certified applicants. 
  • Sufficient knowledge of home health care operations to be able to work, visit patients and follow CVHCare procedures with minimal supervision.
  • Knowledge of or the ability to learn Medicare regulations.
  • Ability to safely assist patients weighing at least 100 lbs, using proper body mechanics in changing bed position, getting up from bed, transferring and walking up to 100 feet as the patient is safely able, with or without devices, and with or without caregiver assistance. 
  • Bilingual skills are a plus, but we offer translation services in more than 150 languages.

We'll expect you to come into the San Ramon or Burlingame office for a monthly meeting (in some situations you can join the meetings virtually), but otherwise you will spend your time in the field at patient homes. Our goal is to minimize your drive time, but you should be prepared to visit patients within 15 to 20 miles of your home. If you do have to travel far, we'll try to assign you to multiple patients in the same area. In Northern California, CVHCare serves patients in Napa, Solano, Contra Costa, Alameda, San Francisco, San Mateo, and Santa Clara Counties.

If you drive to visit patients, you will need a current valid driver's license and a registered, insured vehicle.

The Role

As Certified Home Health Aide, you will report to the Director of Patient Care Services and serve as a paraprofessional member of a home care team where you will take case-by-case direction from a Registered Nurse or Therapist. Typically, you will work with a variety of clinicians as you complete 30 or more visits each week (if full-time). 

Under supervision, you will perform various personal care services as necessary to meet each of your assigned patient’s needs. You will be responsible for observing patients, then documenting and reporting your observations and the care performed. For example, if you are giving a sponge bath and you notice a rash or other marks on the skin, you will pass that information to the appropriate parties.

As Certified Home Health Aide, you will be assigned to patients by a Registered Nurse, Therapist or Case Manager in a manner that promotes quality, continuity, and safety of patient’s care.

In performing your tasks, you will:
  • Provide personal care including:
    • Baths.
    • Back rubs.
    • Oral hygiene.
    • Shampoos.
    • Changing bed linen.
    • Assisting patients with dressing and undressing.
    • Skin care to prevent breakdown.
    • Assisting the patient with toileting activities.
    • Keeping patient’s living area clean and orderly, as appropriate.
  • Assist in feeding the patient, if necessary.
  • Take and record oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency).
  • Assist in ambulation and exercise according to the plan of care.
  • Perform range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
  • Meet safety needs of patients and use equipment safely and properly (foot stools,
    side rails, etc.).
  • Report on patient’s condition and significant changes to the assigned case manager.
  • Adhere to CVHCare's documentation and care procedures and standards of personal and professional conduct.
Note: The home health aide will not function in any manner viewed as the practice of nursing according to the State's Nurse Practice Act. Specifically, the home health aide will not administer medications, take physician's orders or perform procedures requiring the training, knowledge and skill of a nurse, such as sterile techniques.

In general you'll work standard hours, but you also could take a two-hour break between morning and afternoon patients, making up those two hours in the early evening.

Your supervisors are happy to take calls or meet with you to answer care questions, brainstorm ideas or help clear up confusion. Our executives take turns in a 24-hour on-call rotation to make themselves available to you at any time. We'll provide the tools you need, including a smartphone, laptop/tablet, visit bag, clinical equipment and field devices.

Professional development
You'll enjoy paid orientation, which includes extensive classroom and hands-on training, and incorporates a simulation skills lab and 1:1 field training with seasoned peers and clinical management. Our dedicated Clinical Training Department offers ongoing learning, and we pay for continuing education and training. If you have the ambition, and once you've proven yourself in this role, you might set your sights on moving up to a clinical trainer, agency manager or division lead role.
